Correcting a Charting Error
The idea
This case is constructed for this course. It shows how a medical assistant corrects a documentation error and adds a missed entry in the right way.
In practice
At the end of the morning, a medical assistant reviews her entries and finds two problems. First, she recorded a patient's pulse as one hundred twenty when she had measured seventy-two; she had typed it into the wrong field. Second, she forgot to document a wound check she did at ten o'clock. For the error, she does not delete the wrong number. She uses the electronic record's correction function, which keeps the original entry visible, records the correct value, and stamps the correction with her name and the date and time. She adds a brief reason: "entered in error." For the missing note, she adds a new entry labeled as a late entry. It shows the current time of writing and states that the wound check actually happened at ten o'clock. She does not change the entry's time to make it look as though it was written then. Because the wrong pulse could have prompted a concern, she also tells the provider about the correction.
Where people slip
The record's value depends on its honesty. A correction that leaves the original visible shows what happened and who fixed it. An erased or backdated entry, even one that only fixes an innocent mistake, makes the whole record harder to trust, and in a dispute it can look like an attempt to hide something. Correcting openly, the same day, and telling the provider when a mistake could have mattered, is what keeps the record trustworthy for everyone who relies on it later.
