The idea
ICD-ten-CM diagnosis codes have three to seven characters.
In practice
The first characters identify the category, and further characters add detail; some categories need a seventh character, and a placeholder character holds positions when a code is shorter.
Where people slip
A code missing a required character is invalid, and the claim will fail.
