Leading AI at Work: Governance, Rollout and Practice / Unit 5 LAI 122 worked example, 60 to 90 seconds
Recovering From a Failure Mode
The idea
The book gives a recovery for each of the four failure modes, and the steps apply them in turn.
In practice
Step one: for shelfware, shorten the documents toward sharp Silver Packs, link them from stories and data models, and ask for them in key meetings. Step two: for the fortress methodology, return to the core questions: will this help us avoid misunderstandings, and will this make AI use safer? Then loosen non-essential rules and concentrate on Gold things. Step three: for a stale Pack, assign a steward, run a short refit that compares the Pack with the schema, the screens and the policy, and add the thing to the review cadence. Step four: for AI without anchors, name each feature's primary things, update their Packs, document how the AI reads and writes them, and introduce the feature gate.
Where people slip
The fortress is the failure method champions most often cause themselves. Enforcing the method exactly as written, too early, produces resentment that outlasts any benefit; the recovery is to ask less, in fewer places.
