The idea
Designs other than the randomized controlled trial fall short of cause and effect because they carry higher risks of bias. The most they establish is an association.
In practice
A cohort study showing that two things occur together in the same people.
Where people slip
Association is a real finding and a different one. Reporting an association as a cause is the commonest overstatement in the literature.
