Appraisal asks whether a study's findings can be applied directly to particular patients — the ones this decision concerns.
In practice
A finding from a general adult population, weighed for a specific patient with atypical findings.
Where people slip
"Directly" carries the weight. Evidence that needs extrapolation to reach your patient has not been applied directly, and the gap should be stated.
