The Patient Declines the Best-Supported Option
The idea
The technique: hold a decision where the evidence is strong, the clinician agrees with it, and the patient declines, without treating the refusal as an obstacle to be managed.
In practice
A patient at high risk of pressure injury refuses the two-hourly repositioning schedule. The evidence supports it, the guideline recommends it strongly, and the clinician is convinced. Evidence-based practice is stated from the decision rather than from the evidence, and the decision has three components rather than one. External evidence says the schedule reduces incidence. Clinician expertise says this patient's risk is genuine. Patient-valued preferences say that sleep matters more than a risk reduction the patient has understood and accepted. That third component is not a veto to be overcome. It is one of the three sources of highest-quality knowledge, and a decision that discards it is not evidence-based practice at all. The conversation turns to ownership of health rather than to compliance: what the patient wants their days to look like, what they are willing to trade, and what they understand about the risk. Self-efficacy matters here, because a patient who believes the outcome is outside their control participates differently. Appraisal does not guarantee integration, and this is where that gap shows. What is negotiated is not surrender: a modified schedule during waking hours, a redistributing surface overnight, and daily skin assessment with the patient looking too.
Where people slip
Preferences are not consulted at the end, after the evidence has decided. A clinician who presents the schedule and then asks whether the patient agrees has already made the decision and is seeking assent to it. The patient's values belong in the room while the options are still open, and clinicians control the flow of information that makes that participation possible.
