The idea
If the research base is insufficient in quality or quantity, evidence-based practice has nothing sound to translate. If quality data show poor processes, practice fails whatever the evidence said.
In practice
A well-supported change that never worked in a setting whose processes were broken.
Where people slip
Blaming the evidence when the process failed, or the process when the evidence was thin, misreads which part broke.
