The idea
Searches are built as numbered statements: synonyms joined with OR first, then those combined sets joined with AND.
In practice
Two synonym sets each combined with OR, then joined to each other with AND.
Where people slip
The order matters. Joining everything with AND at once collapses the synonyms and loses the studies that used the other word.
