The O in PICOT is the outcome expected: what should change, stated so it can be measured or observed.
In practice
Patient satisfaction with hygiene; perceived confidence; a rate of events.
Where people slip
A question can have more than one outcome, and often should. What it cannot have is an outcome nobody could measure.
