The idea
Euthanasia divides by consent and by means: voluntary or not, and active — bringing death about — or passive, withholding what would delay it.
In practice
A signed refusal of another round of treatment is voluntary and passive; a requested lethal dose is voluntary and active. The debate lives between the pairs.
